New Patient Registration

If you live within our practice catchment area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your post code, so check directly by clicking the above link.

Registering as a Permanent patient - Once you have checked the above link and if you are in the catchment area, please download, print, fill and complete the GMS1 form and The Orchard Surgery Registration form and email these to orchard.surgery1@nhs.net, which will provide us with useful information whilst we wait for your medical records to arrive from your previous doctor. Please use the same forms for registering adults and children. If you are registering a child under 16 years of age, we will need a full list of their vaccines with the forms to complete our registration process at the practice. If the forms are not fully completed and appropriate information is not provided on the forms, this will further delay your registration at the practice. Please also remember to activate your online patient access during the registration period, this helps you to book appointments and request medication at your own convenience.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date you get registered at the practice. Please contact reception for further information.

Registration times and days at The Orchard Surgery

Given the current circumstances with COVID-19, please do not bring the forms in the practice as we are not seeing patients face to face. Once you have filled all relevant forms above, please email it to orchard.surgery1@nhs.net, we are accepting the forms only via email till further notice. Once the registration process has been completed, a member of the staff will be in touch with you. The registration process can take up to 2 weeks.
